Título: Influence of Vendor Strategy and Peer Pressure on the likelihood of achieving a more comprehensive HIT infrastructure. An analysis in the Hospital Setting.

Resumen: No conclusive evidence on which factors can motivate hospitals to enhance their HIT infrastructure or factors that may increase the likelihood of achieving a more comprehensive HIT infrastructure. The purpose of this study is to determine whether choosing certain vendor strategies and facing different levels of peer pressure within the environment have an effect on the likelihood hospitals have of achieving a more comprehensive HIT infrastructure. In particular, to achieve an EHR system capable of meeting the meaningful use criteria determined by the US Federal Government. This study uses a longitudinal design to analyze secondary data from The Healthcare Information and Management Systems Society (HIMSS) data set and the Area Resource File. A fixed effects and random logit models were used to analyze the effect that both vendor strategy selection and environmental peer pressure have on a hospital’s ability to achieve meaningful use. An interaction term between peer pressure and competition was included. A likelihood ratio test was performed to compare the fitness of the models to the data. Overall, 4,799 hospitals were studied in a five-year period (2005-2009) with 19,269 hospital- years. Evidence that peer pressure, munificence and uncertainty increase the likelihood of reaching a higher level of EHR capability was found. Conversely, higher market competition may have a negative effect on the likelihood of increasing EHR capability. On the other hand, vendor strategy and organizational factors are no longer significant after controlling for unobserved heterogeneity, except for affiliation to a multihospital integrated delivery system. Policies to increase the number of hospitals with more comprehensive EHR systems need to acknowledge different types of adopters. Additionally, these types of initiatives may have a multiplier effect.
